This position is with a progressive, financially stable Hospital System.

Salary and Benefits

• Salary Range - $80,121.60 - $120,161.60
• Full-time No Weekends

Responsibilities

• Reports to the Case Management Director
• Responsible for promoting patient-centered care by coordinating all aspects of hospital-based case management for his/her assigned area of responsibility in alignment with the goals of the Case Management Department.
• Manages the case management program including coordination, supervision, and administrative oversight of the team.
• Supervise support staff and ensures case management practices are consistent with hospital policies regulations.
• Monitors the quality of the interactions, documentation, and assessments of the case management staff.
• Coordinates activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization.

Requirements

• Associate’s Degree in Nursing, Bachelor’s Degree in Nursing (2-year grace period) or Master’s Degree in Social Work.
• 3+ years of experience in Acute Care Case Management or Social Work.
• 2+ years of Supervisor experience.
• Clear and active RN license in state of Florida
