Job Overview:

RWJBH is seeking a Social Worker for our Palliative Care Pediatrics Department (PACCT team) at the New Brunswick campus. The Pediatric Palliative Care Social Worker is an integral member of the healthcare team dedicated to providing comprehensive psychosocial support to children with life-limiting illnesses and their families,

The social worker collaborates closely with medical providers, nurses, and other disciplines to optimize the child's quality of life, address emotional and practical needs, and facilitate end-of-life care planning,

Qualifications:

Required:

  • Graduation from an accredited graduate school of social work with an MSW Masters in Social Work degree
  • State licensure required by the NJ State Board of Social Work examiners
  • Licensed Clinical Social Worker LCSW in the state of New Jersey

Preferred:

  • Case Management certification
  • Palliative Care certification preferred

Scheduling Requirements:

  • This is a full-time, day position (40 hours/week).

Essential Functions:

  • Conduct comprehensive psychosocial assessments of pediatric patients and their families to identify emotional, social, and practical needs,
  • Provide individual, family, and group counseling to address grief, loss, anxiety, and other emotional challenges,
  • Collaborate with the healthcare team to develop and implement individualized care plans that address the physical, emotional, and spiritual needs of the child and family,
  • Coordinate discharge planning, including identifying and connecting families with appropriate community resources and support services,
  • Advocate for the child and family s needs and rights within the healthcare system,
  • Educate patients, families, and healthcare providers about pediatric palliative care, end-of-life care, and bereavement,
  • Demonstrate a deep understanding of child development and the unique challenges faced by children with life-limiting illnesses and their families,
  • Provide crisis intervention and support as needed,
  • Maintain accurate and comprehensive documentation of patient and family interactions,
